Daily Screening Questionnaire

As part of our Back to School plan, we are reminding all our students and staff to stay home when sick. 

Students should be screened every day by completing a checklist before going to school and may need a parent or guardian to assist them to complete this screening tool. The Stay Home When Sick Daily Screening Questionnaire will be provided to all our families and students should be screened every day by completing this checklist before going to school.
